רכשתי מכשיר בחו"ל (ResMed S9 Elite™ + H5i™ Heated Humidifier + Mirage Activa™ Nasal Mask מאתר cpap.com).
פניתי לכללית מושלם אודות החזר והם דורשים חשבונית מס בנוסף לקבלה.
מה זה ? 
מאיפה אני משיג את זה ? מה אני אומר לחברים מיוסטון טקסס לספק לי

The claim form and the invoice is all the information on the order that we have to send out. The invoice and the claim form is normally all that is needed to submit to the insurance company for reimbursement.

I'm using the receipt for my refund claim. The insurance claim is not useful for me because my doctor prescription is my proof of need.
What I really need is tax invoice.#194; The form is a commercial document issued by a seller to a buyer, relating to a sale transaction and indicating the products, quantities, and agreed prices for products or services the seller has provided the buyer.
Invoice usually contains :
- A unique reference number.
- Date of the invoice.
- Credit terms.
- Tax payments if relevant.
- Name and contact details of the seller.
- Tax or company registration details of seller.
- Name and contact details of the buyer.
- Date that the goods or service was sent or delivered.
- Purchase order number (or similar tracking numbers).
- Description of the product(s).
- Unit price(s) of the product(s) (if relevant).

- Payment terms (including method of payment, date of payment, and details about charges for late payment).
I have in my hand a receipt : acknowledgment that a specified that a payment has been received.
Since my order is entered directly on your web site, I think the invoice is similar to a order form that produced to the warehouse.
